PATIENT REFERRAL AND TRANSFERS

We are happy to provide continued care for your patients. In order to make the transfer process smooth for your client, we ask that you fill out the transfer form below with the required information as well as email records and recent diagnostics to info@vecmnh.com . Once this is done, please call the hospital to let us know the patient is on the way and discuss the case with the on-staff emergency team.
